For New Jersey children graduating out of the Perform Care system, the link below will take you to a carefully laid out timeline of when you must complete the various steps to transition into the adult system.


Steps to Accessing Services/Supports from the Division of Developmental Disabilities (DDD)

The Division currently has a waiting list for the Community Care Program which provides funding for long term community-based services and supports for people with developmental disabilities. Children that were on the waiting list prior to July 1, 2012 will continue to be serviced by DDD as they age into the adult system and their need for services will be regularly evaluated.
